MEDICAL CLEARANCE FORMS (AFFILIATED STUDENTS)
Agency will need to provide SHIELD with a Medical Suitability Declaration (POST form 2-363) prior to class which shall be signed by a physician. Agency may provide a medical clearance if one has been completed less than 6 months prior to the start of class.

Student will need to provide SHIELD with a Medical Clearance prior to class which shall be signed by a physician. Please reach out to arouland@shastacollege.edu to have a medical clearance form emailed to you.

SHASTA COLLEGE STUDENT ID NUMBER REQUIRED
This course is a FOR CREDIT, college course in partnership with Shasta College. A Shasta College student ID number is required and must be provided on the first day of class. If you do not have a Shasta College student ID number, follow this link to follow the steps to obtain one. You must have your Shasta College Student ID number with you on the first day of class, or you will not be able to attend and will be charged a fee.

OUT OF STATE ATTENDANCE
Out of state fees may apply if you are not a California resident. Please contact Training Director at (530) 768-1348 for questions.

ATTIRE
Regular Classroom Days (M-Th): Business casual attire
Defensive tactics training (Th-F): Tennis shoes /athletic shoes, loose fitting clothing, preferably sweatpants and a t-shirt.

REFUND POLICY
STUDENTS:
– Full refund if student withdrawals prior to September 16, 2024
– 50% refund if student withdrawals between September 17 – September 29, 2024
– No refund provided if student withdrawals after September 30, 2024 or does not attend class.

AGENCY SPONSORED STUDENTS:
– Agency will not be billed for attendance if student is withdrawn prior to September 23, 2024.
– Agency will be billed for 50% of registration fee if student is withdrawn between September 24 – September 29, 2024
– Agency will be billed for 100% of registration fee if student is withdrawn after September 30, 2024 or does not attend class.
** Substitutions or transfer of the reservation (roster spot) to another student or agency is permitted upon approval of the Training Director.

CANCELATION POLICY
SHIELD reserves the right to cancel any class due to unforeseen circumstances or low enrollment.
